The Tennessee Medical Association is no longer accepting nominations for the 2012 Community Service, Distinguished Service and the Outstanding Physician Awards. Thank you for your interest.
How to Nominate
Each nomination should be accompanied by complete information according to the criteria, as outlined in the nomination packet.
A checklist is included for each award category and must be submitted at the time of nomination.
Please submit all nominations and accompanying materials electronically, either in a Word document or as a PDF file. Submission instructions are included in the nomination packet.
